1. Confirm the department/institute is already equipped with personnel to manage the laboratory animal facilities.
2. Confirm that the animal facilities possess the necessary equipment and management approved by IACUC. If the animals which the applicant intends to use need to be fostered at the animal facilities or center of another institution for a particular reason, a documentation proving agreement from said institution must be provided.
3. Fill out the NTU Main Campus Application for Animal Testing and the NTU Proof of Review for Animal Testing Cases For the purpose of case review, if the methods related to the handling of laboratory animals in the project are the same or similar to previously published literature, and said journal complies with relevant laboratory animal protection policies, please attach the literature for the committee's reference (preferably from the last five years).
4. After the application form is signed by the person in charge of the animal facility and the department/institute chair, it shall then be sent to the Research Ethics Office of ORD (2nd floor, Si-Yuan Hall, Shui-Yuan Campus).
5. Review is undertaken by NTU's Animal Experimentation Proposal Review Committee.
6. After examination and approval, the NTU Agreement for Case Review of Animal Testing Project will be granted.

3. One copy of the "NTU Main Campus Application Form for Educational Animal Testing" shall be filled out for each department/institute (incl. Attachments 1 and 2). For the purpose of case review, if the methods related to the handling of laboratory animals in the project are the same or similar to previously published literature, and said journal complies with relevant laboratory animal protection policies, please attach the literature for the committee's reference (preferably from the last five years).

6. Each course need only be applied once. If alterations must be made, please fill out the "NTU Application for Altering of Animal Testing Details".
7. If a new course is opened, please also fill out the "NTU Application Form for Educational Animal Testing (incl. Attachments 1 and 2). The application procedure is the same.
8. Time limit for approval of educational application cases shall not surpass 5 years. If the deadline needs to be extended further, please send documentation for another review.
Application procedures for simple review
Conditions for applying:
1. Projects whose samples are obtained through standard medical practices and are not subject to additional procedures from the animal testing, with the applicant not belonging to the medical team, are eligible for a simple review.
2. Projects which only alter their committee-approved title and do not change the rest of their content are eligible for a simple review. The application form, approval notice, and original relevant documents must be attached to the application for committee members' reference. Applicants also must agree to withdraw the previously approved application.
Review procedures:
After passing review from a veterinarian and a committee member, followed by review from the convened committee members, the case shall then be submitted to the committee for ratification.
