Application for Case Modification

1. Modifications of principal investigator, project name, or framework of experiment are deemed as a new application and the application form must be filled out again.

2. For the following changes, please fill out the "NTU Application for Altering of Animal Testing Details", and modify the content you want to change based on the originally approved online application. 

Items and Procedures Regarding Modification of Applications:

1. If the content of the change is a minor amendment, it only needs to be submitted for administrative review and offer an explanation for the change:

• The period of execution may only be extended to a maximum of 1 year.
• The animals required for the experiment before and after the change shall belong to the same strain.
• The change in the number of animals of the same strain shall be less than 40% of the number indicated in the originally approved plan.
• Changing the source and/or feeding location of the animal.
• Changing, increasing, or decreasing personnel.
• Replacing the medication used for animal disposal or anesthesia which are less harmful and comply with regulations.
• Increasing or replacing non-invasive test sampling procedures or frequency (e.g. 1. replacing heating patches with heating lamps; 2. tracking temperature and weight of animals after completion of the experiment; 3. collecting more feces/hair, etc.)

2. If the changes belong to major revisions, the project must be submitted to the review committee for re-examination:

• Changes in animals required in the experiment (change to a different species, or change in quantity which is more than 40% of the originally approved number)
• Adding procedures concerning bio-hazardous experiments
• Adding procedures concerning invasive experiments
• Experiments involving animal welfare concerns

Modification of Applications

1. Modifications of principal investigator, project name, framework of experiment, or more animal use as the result of adding treatment groups: please fill out the application form again.

2. Minor amendments to the plan's content, such as:

1. extending the period of execution
2. changing personnel
3. changing the animals required for the experiment
4. changing the content, method, dosage, or steps in the experiment, etc.

The procedure to modify an application is the same as applying for a project plan and both shall be submitted in advance. After the modifications are approved and an agreement form is obtained, the experiment may finally be executed.
